All you need to get started is to fill out our Account Set-up Form and scan and email it to

We will then email you the Disclosure and Release Form that must be filled out and signed by each applicant.
You’ll then either fax or scan and email the Disclosure and Release Form to us and we will submit the search(s) immediately.
You will be emailed the reports as soon as they’re received.
